Sagging breasts most often occur with age, after significant weight loss, after one or more pregnancies with (or without) breastfeeding, or as a result of the passage of time. Mammary ptosis occurs when the breast is too low compared with an “ideal” situation.

Often, the nipple and areola are too low and can point downwards, the upper part of the breast becomes empty, and there is a loss of breast shape. The breast appears partially ‘emptied’ and patients wish to regain their body and in particular their décolleté.

A breast lift without prosthesis is an operation that lifts, firms and reshapes the breast without the addition of a foreign body (prosthesis). However, it is possible to combine a breast lift with breast augmentation using a prosthesis or lipofilling.

What does a breast lift without prosthesis involve?

A breast lift or mastopexy is an aesthetic procedure that lifts the breasts, repositioning the areola and reshaping and firming the décolleté – in other words, a breast lift without a prosthesis.

A breast lift without prosthesis is indicated when there is :

  • A sagging breast that has lost its initial shape and firmness, as if ‘emptied’.
  • Areola and nipple pointing downwards
  • Breast asymmetry

The aim of the operation is to reshape the breast by acting on the glandular tissue and excess skin. The breast lift allows the areola to be repositioned and lifted, and the volume to be concentrated in the upper part of the breast in order to restore the natural shape of the breast and make it firmer.

Depending on the objective of the ptosis treatment (sagging and empty breasts, asymmetric breasts, overly voluminous breasts) and the quality of the patient’s skin, the scars from the operation will be different:

  • Scar around the areola only
  • Scar around the areola and vertical scar
  • Scar around the areola, vertical scar and horizontal scar in the submammary fold (T-shaped scar).

The operation lasts 1 hour 30 minutes. It is performed under general anaesthetic during outpatient hospitalisation (discharge the same evening) or during a short stay in hospital (1 night). The mammary areola is lifted and the breast reshaped.

How do I prepare for a breast lift without a prosthesis?

Two to three consultations are necessary to define a surgical plan before an operation, in order to determine with Dr Azuelos whether a breast lift without a prosthesis is the right solution for you. This is the ideal time to understand your history and your expectations of the operation.

Depending on the degree of sagging, a localised scar around the areola (for moderate sagging) or a marine anchor scar (for significant sagging) will be discussed.

The breast lift may be performed alone or in combination with breast augmentation by prosthesis and/or fat transfer.

A mammogram is required before any breast operation. Photographs are taken to simulate the results.

What are the after-effects of a cure for breast ptosis or a breast lift?

The result is immediate: the areola is lifted and the breast is fuller. Post-operative pain is slight and short-lived. The scars are washed daily by the patient and/or a qualified nurse. They fade in 3 to 6 months.

A support bra must be worn for 4 to 6 weeks. Post-operative rest for a few days is recommended.

Sport is not recommended for 1 to 2 months.

This operation does not prevent pregnancy, but it may limit the amount of breast-feeding.

What are the risks of a breast lift without a prosthesis?

The risks of this operation can be summarised as the general risks of all operations (haematoma, infection, scarring problems and anaesthetic risks). One of the undesirable effects of this procedure may, in some cases, be a change in the feel of the breast areola.

What is the treatment for a breast lift?

Even in cases of massive weight loss or multiple pregnancies, this operation is not covered by social security. It is purely cosmetic surgery.

Frequently asked questions

Nipple sensitivity may decrease after surgery and gradually reappear several weeks afterwards. It depends on the degree of ptosis, but one of the undesirable effects of this operation may be a change in the sensation of the nipple or breast areola.

It is entirely possible to increase breast volume with a prosthesis or by injecting one’s own fat following a breast lift, either during the breast lift procedure, to increase the shape, or after the procedure if the patient feels like it.

The result is definitive insofar as no foreign body such as a prosthesis has been added that needs to be replaced, but it is not totally definitive because the effects of time will affect the post-operative result in the same way as it did before the operation. Other factors that could potentially affect this result are weight variations, prolonged exposure to the sun (which induces skin slackening), pregnancy and smoking.
