Breast hypertrophy (overly large breasts) can cause neck, back and shoulder pain, and sub-breast irritation that can develop into mycosis. It can also cause discomfort when dressing, playing sports or engaging in daily activities, sagging breasts and cosmetic discomfort.

The aim of breast reduction surgery is to reduce the volume of the breast (gland) and fat, and to lift and reshape the breast.

Breast reduction surgery

Breast hypertrophy is defined as a large breast volume with physical and/or psychological repercussions. It can appear at puberty, after weight gain or over time. It is often accompanied by breast ptosis, i.e. sagging breasts.

Breast reduction surgery or reduction mastoplasty makes it possible to:

reduce the volume of the breasts
restore the areola and nipple to their correct position
recreate the shape and harmony of the breast Breast reduction also corrects any breast asymmetry. It can also be combined with a breast lift. Breast reduction is possible from the end of puberty and throughout life. In extreme cases, breast reduction is even possible before the end of puberty.

Pregnancy is entirely possible after breast reduction and breast lift surgery. Although breastfeeding is often still possible, it may become less abundant, and the risk of total or partial impossibility of breastfeeding should therefore be considered by the patient. A breast reduction and breast lift does not increase the risk of breast cancer, and may even reduce it.

How do I prepare for a breast reduction and breast lift?

Two or three consultations will be necessary to define a surgical plan.

First of all, you will need to reach a stable weight. Breast size also varies with weight.

The treatment plan will be drawn up according to the patient’s wishes and anatomy. A target volume/cup size will be considered.

Breast asymmetry is sought and will be corrected during surgery.

The position of the scars may vary depending on the project. They can be positioned around the areola and vertically downwards (lollipop) for slight hypertrophy. For medium and large hypertrophies, the scars will be positioned around the areola and vertically downwards in the sub mammary fold (anchor shape). The length of the horizontal sub-breast scar depends on the extent of breast hypertrophy.

As the breast lift uses the same approach, no additional scarring will be necessary.

A mammogram and bilateral breast ultrasound are imperative before any breast surgery in order to have a reference for future mammograms and to detect possible unnoticed breast lesions.

It is essential to stop smoking one month before and one month after the operation in order to obtain the best possible scar quality.

What does breast reduction and breast lift surgery involve?

The operation lasts 1 hour 30 minutes. It is performed under general anaesthetic during outpatient hospitalisation (discharge the same evening) or a short stay in hospital (1 night). The areola is lifted, the breast lightened and reshaped.

The first dressing is made by Dr. Azuelos in the operating theatre and will be removed by him the day after the operation at the consulting room in the case of outpatient surgery (discharge the evening before) or before leaving the clinic if you have slept overnight at the clinic.

What happens after a breast reduction and breast lift?

As soon as the operation is over, the result is visible. The areola is lifted, the volume of the breast is reduced, the shape is rounded and neck and back pain disappear within a few days. Initially, the breast is lifted, shapely and even firm.

Post-operative pain is slight and short-lived. There may also be some post-operative swelling and bruising in the first few days and weeks. No drain is left in place after the operation.

Post-operative care is relatively simple. The patient and/or a qualified nurse will wash the scars daily with soap and water, then a dressing will be applied to the scars. The scars will fade in 3 to 6 months.

A support bra should be worn for 4 to 6 weeks. This will limit post-operative swelling, but will also keep the breasts in good position and relieve tension on the scars. Post-operative rest for a fortnight is recommended. Sport is not recommended for 1 to 2 months.

After a few months, the breast unwinds and takes on a natural appearance. In addition to the appearance of the breast, the long-term impact on quality of daily life, back and neck pain, sporting activities and weight balance is significant and stable over time.

The operated breast remains sensitive to the hormonal cycle and to weight variations. It is therefore important not to put on too much weight, as this could lead to an increase in breast volume.

This operation does not prevent pregnancy, but it may limit the amount of breast-feeding. Apart from painkillers, no other type of medication is necessary during the postoperative period.

What are the risks of breast reduction and breast lift?

It often happens that the sensitivity of the mammary areola changes, with numbness, insensitivity or unusual sensations of the areola and nipples. These changes are frequent and disappear gradually, at least partially, over time (several months or years depending on the case). They may, however, be permanent.

Complications of breast reduction and lifting are fortunately very rare.

These include the risks associated with general anaesthesia, haematomas and exceptional infections.

Haematomas: these are rare, but may require monitoring if they are small or evacuation if they are larger.

Infections: infrequent, they may require antibiotic treatment as well as drainage and surgical lavage.

What is the cost of a breast reduction and breast lift?

The procedure may be covered by Social Security and your health insurance company, provided that each breast is reduced by 300 grams (1 to 2 cups).
