Breast augmentation by adipocyte grafting, lipofilling, lipomodelling, liposculpture or implant-free breast augmentation; all these terms refer to the same procedure, which consists of breast augmentation without implants.

This procedure consists of increasing the volume of the breasts by using excess fat from different areas of the body, i.e. the stomach, hips, thighs, inner knees, back, etc., in order to gain volume without foreign bodies, while at the same time enabling the patient’s figure to be refined overall (liposuction of areas of excess fat). The result is natural and harmonious both to the touch and to the eye.

What is breast augmentation without implants?

Initially developed to correct breast malformations and then used in breast reconstruction, breast augmentation using adipocyte grafting offers a number of advantages:

It combines the correction of a small breast volume with an excess of fat from another part of the body;
It does not involve a foreign body (prosthesis) that could be rejected. The fat used in breast augmentation without implants is the patient’s own;
It can be repeated for a new augmentation;
It does not require re-operation (unlike breast augmentation with a prosthesis, which has to be changed after 10 years);
Breast augmentation with adipocyte grafting can be performed alone or in addition to breast augmentation with implants and/or a breast lift.

Breast augmentation without implants

Adipose tissue (fat) is removed by liposuction, then treated and purified before being transferred to the breasts (self-grafting). This surgery does not leave any large scars (the cannula entry holes).

This technique is particularly indicated for patients wishing to increase the volume of their breasts slightly to moderately, or to correct breast asymmetry, or as part of reconstructive surgery (following breast cancer or breast malformation). However, lipofilling or breast augmentation without implants requires sufficient fat reserves, which is why very thin patients are not good candidates for this procedure.

This procedure is also indicated for patients who want a breast augmentation without implants, without foreign bodies, and who wish to avoid the risks associated with the appearance of periprosthetic ‘shells’. The duration of the implant-free breast augmentation procedure depends on the sites to be removed, the amount of fat tissue to be transferred and the lipomodelling.

How should I prepare for breast augmentation without implants or by lipofilling?

Before a breast augmentation without implant, two to three consultations are necessary to define a surgical plan, define the fat donor zones, the desired volume of the augmentation and define whether a complementary operation (augmentation by prosthesis or breast lift) is desirable. The consultation also allows Dr Arié Azuelos to check that there are no abnormalities or pathologies of the breasts. A mammogram is required before any breast operation.

Dr Arié Azuelos will systematically take photographs of the patient.

Strict cessation of smoking for at least one month before and after the operation is necessary to improve the uptake of fat grafts and better vascularisation of the breast, and to optimise healing. It is also necessary to stop taking aspirin, anti-inflammatory drugs or oral anticoagulants in the 15 days prior to the operation to reduce the risk of haemorrhage during the operation.


Finally, it is recommended that you stop taking oral contraceptives 1 month before the operation in order to reduce the risk of thromboembolism inherent in taking this type of contraceptive.

How does breast augmentation without implants work?

This procedure is carried out under general anaesthetic during outpatient hospitalisation (discharge on the same day as the operation).

In the case of major liposuction (more than 3 litres in the same operation), an overnight stay in hospital is recommended. The length of the operation depends on the extent of the liposuction and lipofilling.

Liposuction is performed in areas of excess fat (these areas vary from patient to patient), but in the majority of cases on the abdomen, buttocks, inner thighs, ‘love handles’ and saddlebags, using a fine cannula with low suction power, which slowly and evenly sucks up the fatty tissue, without creating irregularities in the suctioned areas. Scars are very discreet and are usually hidden in natural folds and underwear.
The next stage consists of filtering and treating the fat removed to isolate and preserve good quality fat cells and eliminate impurities and unusable fat.
Finally, the fat is reinjected into each breast using a round-tipped cannula.

What are the after-effects of breast augmentation by lipofilling/fat transfer?

An increase in the size of the breasts is visible from the end of the operation, but will not be definitive for another month. The breast will then evolve normally with time, weight loss and pregnancy.

Bruising and oedema appear in the liposuctioned areas and on the breasts. The bruising disappears in about ten days, while the oedema, if significant, may persist for 2 to 3 months in the suctioned areas.

It is quite normal to notice a partial loss of breast volume following the operation, linked to the disappearance of post-operative oedema. It is also possible, as with any reinjection of natural fat, to see a resorption of around 30% during the first few months. Very rare irregularities can be corrected later under local anaesthetic.

Dressings are simple; scars are washed daily by the patient and/or a qualified nurse.

Stockings and compression garments should be worn in the suctioned areas for 4 weeks.

Post-operative pain is moderate and mainly localised to the areas removed. Post-operative rest for a few days is recommended. Sport is not recommended for 1 to 2 months. This operation does not prevent pregnancy or breast-feeding, although it is preferable to space out pregnancy following this operation to allow the result to stabilise.

The fat transferred remains for life and will behave like body fat, losing volume if you lose weight and vice versa.

What are the risks of breast augmentation without implants?

Apart from the complications common to all operations (haematoma, infection and complications linked to anaesthesia), the major risk of this operation is the resorption of part of the fat transferred. Indeed, as with any graft, some of it may ‘not take’.

There is no increased risk of breast cancer.

How is breast augmentation without implants covered?

In exceptional cases, this operation is covered by Social Security +/- your mutual insurance company.

With the exception of cases of agenesis, breast malformation or cancer, this is a cosmetic operation.

The increase can be small to moderate depending on the patient’s morphology (generally between 1 and 1.5 cup sizes larger, although it is possible to repeat this procedure). During the consultation, depending on the patient’s body (whether or not there is enough fat available) and her expectations, we can decide whether this technique is right for her. There are also some contraindications to breast lipofilling, particularly if the patient is underweight.

Breast lipofilling in no way prevents the patient from breastfeeding.

In principle, this operation is not reimbursed by social security, except in cases of breast hypotrophy or asymmetry, or in cases of breast reconstruction after cancer, to correct the after-effects of lumpectomy. Depending on the case, a prior agreement may be given if the patient is in one of the above-mentioned situations.

As fat is removed by liposuction from fatty areas prior to breast lipofilling, this operation is carried out exclusively under general anaesthetic.

Breast lipofilling or breast augmentation without implants provides a definitive result insofar as the injection of fat is definitive, unlike breast augmentation with implants, which requires the prostheses to be changed every 10 years. However, as with any operation involving the addition of natural fat, it is possible to observe a reduction of around 30% during the first few months after the operation.

It should also be noted that the result of this operation is subject to variations in the patient’s weight, so if the patient loses weight, the volume of the breasts may be affected.

Breast lipofilling or breast augmentation without implants was initially banned due to fears that the injection of fat could trigger breast cancer. Since 2011, this procedure has been scientifically validated by the French Society of Plastic, Reconstructive and Aesthetic Surgery.
