It’s just a few centimetres, but they make a big difference. The size and circumference of the penis are often a source of complexes, commonly known as the ‘locker room complex’, which can have major repercussions on psychological health, sexuality and social relationships.

Surgery provides a rapid and appropriate response to these complexes through aesthetic penoplasty or phalloplasty, an operation designed to increase the size of the penis in both length and circumference.

For these reasons, recourse to surgery has increased considerably over the last ten years or so, making penoplasty one of the leading procedures in male surgery.

What is a penoplasty?

Penoplasty is an operation to improve the aesthetic appearance of the penis.

When it is used to treat malformations of the penis (micropenis; penile deformities: kinks, curvatures, etc.), it comes under the heading of reparative penoplasty.

When it is used to improve the aesthetic appearance of the penis, but is not a reparative penoplasty, it is an aesthetic penoplasty.

There are 3 types of penoplasty, whether cosmetic or reconstructive:

  • Lengthening penoplasty: this procedure lengthens the penis by a maximum of 2 to 3 cm.
  • thickening penoplasty: this procedure increases the diameter of the penis by a maximum of 30 to 50%.
  • Complete penoplasty: this procedure consists of performing a lengthening and thickening penoplasty at the same time.

Penile thickening

Penile thickening is performed by lipofilling, i.e. by reinjecting the patient’s own fat under the shaft of the penis to obtain circumferential thickening.

Several lipofilling sessions are possible.

Penile lengthening

Penile lengthening is performed in 2 stages:

  • Skin advancement: a V or VY incision is made, depending on the case, hidden in the pubic hair.
  • Section of the suspensory ligament of the penis: the ligament attaches the dorsal surface of the penis to the pubic bone, which means that sectioning it allows the part of the penis buried behind the pubis to come out and thus obtain penile lengthening.

The operation takes about 1 hour. It is usually performed under general anaesthetic combined with a local anaesthetic to reduce post-operative pain.

Complete penoplasty

This is a combination of the two previous techniques: penile lengthening and penile thickening.

How do I prepare for a penoplasty?

Before the operation, two consultations are necessary to define the surgical plan. During these consultations, Dr Azuelos analyses and advises the patient. He will also take photographs of the patient.

Smoking must be stopped one month before and after the operation to reduce the risk of complications and to optimise healing.

It is also necessary to stop taking certain medications, such as anti-coagulants and anti-inflammatories, one week before the operation.

What happens after a penoplasty in Paris?

The patient is usually hospitalised for one day and discharged the same evening (ambulatory). Sometimes the hospital stay lasts overnight, particularly when the patient does not have a close relative available for discharge from the clinic.

The sutures are absorbable, so there is no need to remove them.

During the first weeks or months following the operation, the following symptoms are generally observed:

– Swelling (oedema) of varying severity in the penis, which subsides and disappears within a few days.

– A temporary reduction in penile sensitivity.

– Bruising, which fades 15 to 21 days after the operation.

– Post-operative pain is moderate and mainly localised to the areas removed. Local anaesthesia can significantly reduce this pain, in conjunction with painkillers following the operation.

Urination is resumed immediately and sexual intercourse is generally resumed 3 weeks after the operation.

It is possible to resume sport 15 days after the operation, wearing suitable underwear.
A few days’ post-operative rest is recommended. The period of professional interruption varies between 2 and 5 days.

The result is immediate. However, the final result is obtained after 3 months, when the oedema has completely disappeared.

In the case of a thickening penoplasty, the result is not seen for 6 months, because following the disappearance of the oedema, it is possible, in some patients, to observe a maximum resorption of 30% of the fat injected. When this happens, a second lipofilling session may be considered, depending on the case.

What are the risks of penoplasty?

As with any surgical procedure, penoplasty presents risks, the frequency of which is rare but worth mentioning.

– Infection is rare, but possible, and requires drainage and antibiotics.

– A change in sensitivity: numbness or insensitivity at the base of the penis may occur, although this is generally temporary.

– Reduction in the obliquity of the erect penis.

Is penoplasty reimbursed by social security?

When the patient has a genuine micropenis that requires lengthening by removal of the part of the penis hidden behind the pubis, then this operation may be covered by social security.

However, purely cosmetic lengthening and penile thickening are not covered by the health insurance scheme.

In the case of penile thickening by lipofilling, the increase in penile circumference depends essentially on the quantity of fat removed and reinjected, but also on what was decided during the consultation. It is in the region of 2 to 3 cm in circumference.

With lengthening penoplasty, the increase in penile length is around 3 cm. Weight variations can influence the result of penile thickening upwards or downwards by around 30%.

For penoplasty enlargement and lengthening, the result can be considered definitive once the oedema (swelling) has subsided. However, it should be noted that in some cases the result of penile thickening by lipofilling may vary due to natural resorption or variations in weight. In the case of weight variation, the result may vary upwards or downwards depending on your weight gain or loss.

The variation in the result of thickening by lipofilling linked to natural resorption is of the order of 30% maximum. When this occurs, a second lipofilling can be performed to perfect the result.

Sexual relations must be suspended for approximately 3 weeks post-operatively. Sensitivity will gradually return to normal after 2 to 3 months.
